1. Agreement to Terms
By creating an account or using the FormAI service ("Service"), operated by DBM-AI ("we", "us", "our"), you ("Customer") agree to be bound by these Terms of Service. If you do not agree, do not use the Service.
2. What FormAI Does
FormAI is an AI-powered form assistant that integrates with your website forms via a JavaScript plugin. The Service provides:
- Per-field AI help -- contextual tooltips and chat-based guidance for form fields.
- Voice input (STT) -- speech-to-text so users can fill forms by voice.
- Text-to-speech (TTS) -- reads field descriptions aloud for accessibility.
- Auto-fill suggestions -- AI-generated field values based on conversation context.
- Multi-language support -- form assistance in 74+ languages.
- PII tokenization -- recognized personal data patterns (emails, phones, IDs, cards) are replaced with safe tokens in the browser before reaching our servers. Free-text names and addresses may not be detected.
3. Accounts & Registration
- You must provide a valid email address and verify it to activate your account.
- You are responsible for maintaining the confidentiality of your password and API key.
- You must not share your API key or allow unauthorized access to your account.
- One account per person or organization. We may suspend duplicate accounts.
- You must be at least 18 years old or have legal authority to enter into this agreement on behalf of your organization.
4. Free & Paid Plans
- New accounts start on a free plan with limited monthly form sessions.
- Paid plans are billed monthly or annually via Stripe. Prices are displayed on our pricing page.
- Usage limits (form sessions per month, number of forms, number of domains) are enforced per plan. When limits are reached, the assistant is paused until the next billing cycle or plan upgrade.
- We reserve the right to modify pricing with 30 days' notice to existing customers.
- Refunds for paid plans are handled on a case-by-case basis. Contact support within 14 days of a charge to request a review.
5. Acceptable Use
You agree not to:
- Use the Service for any unlawful purpose or to facilitate illegal activities.
- Attempt to reverse-engineer, decompile, or extract the source code of the plugin or API.
- Submit malicious content, inject code, or attempt to exploit the API or plugin.
- Use the Service to collect, store, or process personal data in violation of applicable privacy laws.
- Exceed your plan's usage limits through automated means, API abuse, or circumvention.
- Remove, hide, or tamper with the "Powered by" branding on free/standard plans (unless your plan includes white-label rights).
- Resell, sublicense, or redistribute the Service without written permission.
6. Data Processing & Privacy
- The plugin applies pattern-based tokenization to personal data (PII/PHI) in the end user's browser before it reaches our servers. This is a best-effort measure; unstructured personal data (names, addresses, free-text descriptions) may not be detected and could be transmitted to our servers and AI providers.
- Conversations are ephemeral and are not stored on our servers after the session ends.
- We store your account information (email, company name, hashed password) and usage metrics.
- We do not sell, rent, or share your data with third parties for marketing purposes.
- AI processing is performed by third-party providers (e.g., OpenAI). Only tokenized, non-personal data is sent to these providers.
- As the website owner, you remain the data controller for your end users. You are responsible for informing your users that AI assistance is available on your forms.

7. Service Limitations
- AI accuracy: The AI assistant provides suggestions, not guarantees. Responses may be incorrect, incomplete, or contextually inappropriate. Users should always review AI-generated field values before submitting forms.
- Availability: We aim for high uptime but do not guarantee uninterrupted service. Maintenance windows, infrastructure issues, or third-party outages may cause temporary unavailability.
- PII detection: While we detect 20+ ID formats and common personal data patterns (emails, phone numbers, government IDs, credit cards, IBANs, dates of birth), tokenization is best-effort. Names, physical addresses, and free-text descriptions of personal information may not be detected by pattern matching. Server-side logs apply additional masking before writing to disk.
- Browser compatibility: The plugin requires modern browsers (Chrome, Firefox, Safari, Edge). Older browsers may not support all features (TTS, STT, Shadow DOM).
- Form compatibility: The plugin works with standard HTML forms. Heavily customized or non-standard form implementations may not be fully supported.
- TTS processing: Text-to-speech features send text to Microsoft's edge-tts service for on-demand audio generation. TTS audio is streamed directly to the user and is not cached or stored on our servers.
8. Intellectual Property
The Service, including its code, design, documentation, and branding, is owned by DBM-AI. Your use of the Service does not grant you ownership of any intellectual property. You retain ownership of your website content and form data.
9. Account Termination
- You may delete your account at any time through the Account settings or by submitting a deletion request.
- We may suspend or terminate accounts that violate these terms, abuse the Service, or remain inactive for more than 12 months.
- Upon termination, your data (account information, form registrations, domain configurations) will be permanently deleted within 30 days.
- Active paid subscriptions will be canceled and no further charges will be made.
10. Limitation of Liability
To the maximum extent permitted by law, DBM-AI shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including loss of profits, data, or business opportunities, arising from your use of or inability to use the Service. Our total liability shall not exceed the amount you paid for the Service in the 12 months preceding the claim.
11. Disclaimer of Warranties
The Service is provided "as is" and "as available" without warranties of any kind, whether express or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, and non-infringement.
12. Changes to Terms
We may update these terms from time to time. Material changes will be communicated via email or a notice in your dashboard. Continued use of the Service after changes constitutes acceptance of the updated terms.
13. Governing Law
These terms are governed by the laws of the European Union and Romania. Any disputes shall be resolved in the courts of Romania, unless otherwise required by applicable consumer protection laws.
